The Hmong Health Navigation Project

 

As a community based organization which has assisted refugees, immigrants, and new Americans for over 15 years, Fresno Center for New Americans (FCNA) understands that the newly arriving Hmong refugees will face many challenges, specifically to health access issues.  To address some of these issues, FCNA has implemented the Hmong Resettlement Health Systems Navigation Project (The Hmong Health Navigation Project).

 

The Hmong Health Navigation Project is funded by The California Endowment (TCE).  Phase 1 of the project has ended in March 2006.  The success of Phase 1 persuaded TCE to fund this project for two more years.  The goals of phase II are to 1) build and maintain strong relationships with elected and appointed officials, private healthcare systems, and the Hmong Health Collaborative; 2) yield new funding opportunities and policy changes that benefit the Hmong community; 3) increase representation of the Hmong community in the health care workforce; and 4) promote greater Hmong participation in health careers to boost the number of Hmong-literate health workers in California.

 

Services provided are but not limited to:

  • Information & referral services
  • Linkages with primary care providers,
  • Interpretation & translation services related towards health,
  • Others
